- If it says "On Hold" next to the lab on your protocol sheet, do not download those instructions.
- At the bottom of the email, there will be videos to help guide you as well.
- If a "Blood Test" was ordered for you, please note the requisition form will be attached at the bottom of your emailed invoice. A separate confirmation email will be sent as well along with a link that will allow you to change the selected patient service center location if needed. Please don't forget to bring your requisition with you to the patient service center.
- Please keep the tracking information, and make sure the labs receive your samples.

For International patients, please follow the instructions below:
- DUTCH TEST can be returned by whatever shipping method guarantees delivery to the US within 7 days, at your preferred courier at your expense. You will need the commercial invoice. These forms are helpful to independently ship this kit and in addressing concerns with Customs. This is to be included in the return shipment with the kit and requisition from. The requisition form is online at the member's area. You can click the direct link to the forms you need from the above list. Send the forms back to the lab, with your samples. See protocol sheet for the exact name to look up.
- DSL GI-MAP TEST. can be returned by whatever shipping method guarantees delivery to the US within 6 days, at your preferred courier at your expense. You will need the forms- manufacturer's declaration and commercial invoice. These forms are helpful to independently ship this kit and in addressing concerns with Customs. They are to be included in the return shipment with the kit and requisition from. The requisition form is online at the member's area. You can click the direct link to the forms you need from the above list. Send this form back to the lab, with your samples. See protocol sheet for the exact name to look up.
- GENOVA ORGANIX URINE KIT can be returned at your preferred courier at your expense. See attached customs forms needed to send back. You will need these forms: Manufacturer's Declaration form, 3 copies of the Commercial Invoice. This should be sent with the kit when it is shipped back to the US for testing. These should be sent with the kit in an accessible envelope placed on the outside of the Fed Ex mailer. Both of these forms are helpful in addressing concerns with Customs.
- MITOSWAB. Kindly make sure to 1) Freeze packs over night before swabs are taken. 2) Dispatch kits only between Monday through Thursday so that the lab will get it during weekdays. 3) Choose the overnight delivery option and should be kept with ice packs (well frozen) to make sure that swabs will remain active when they arrive at the lab.
